Responsibilities

The Transition Care Coordinator (Medical Assistant) works as a member of the health care team to promote timely, comprehensive, coordinated, cost-effective, and efficient quality care to all patients. This position is responsible for transition of care activities related to inpatient and outpatient healthcare facilities. It is a telephonic role to provide transitional care coordination for patients, family and caregivers who receive primary care health services from an HQMP provider. The primary goal of this position is to ensure patients understand discharge instructions, medications, and proper follow up.

The Transition Care Coordinator works in collaboration with the Care Managers, Practice Transformation staff, and primary care practice staff. Population served based on scope of services in the department.

 

Qualifications

High School Diploma or equivalent experience.
 

PREFER: 1 Year Physician Office Experience with extensive clinical experience,Current Medical Assistant
 

License, Registration, or Certification Requirements:

BLS Certification

 

 

M - F 7a - 6p varied schedule
